Welfare Visit

Do you have a family member or loved one that lives alone or maybe you live alone and would benefit from a nurse visit. 

If you are worried about your health or the health of a loved one and would benefit from a full set of observations, advice and sign posting for health problems, then this service is for you. 

One of our nurses will visit your loved one or yourself at home. At the 30 minute appointment the nurse will complete a full set of nursing observations, which includes, Blood Pressure, Oxygen Saturations, Heart Rate, Breathing Rate and Temperature and act accordingly on the results. This may include contacting your GP, communicating with family or just for keeping a record for monitoring. We will ask a range of questions about your health and wellbeing. We can also perform a urine dip stick if it is felt necessary. 

The nurse will then create a care plan that includes your wishes for any follow up appointments or referrals needed. With consent the results of these appointments can be communicated with family, professionals or escalated to appropriate services if necessary. 

This is completely up to you. We can visit as often or as little is required. You may just want a one off check up to alleviate any concerns for a loved one, or you may require weekly or more frequent visits. We strive to meet your needs. 

If you are struggling to get the level of care you need through other services or you would like top up visits in between NHS visits then the Independent Nurse is a great option. 

We have a small team of nurses meaning we can offer continuity of care and reliability of who will visit you and a set time. 

Out team comprises very experiences nurses who have been nursing for a long time and competent to deal with any needs that arise. We work closely with other professionals and liaise with your GP  or other services when needed to ensure you are accessing the right services.
